MINE ID # OR PROSPECTING ID # M-83-090 INSPECTION DATE 05/14/99 INSPECTOR'S INITIALS ESC The following problems (PB) and/or potssible violations (PV) (and suggested corrective actions) were identified during this inspection. The problems should be corrected by the dates given, or they will become possible violations. The inspector noted on the previous page should be notified of all corrective actions taken. PROBLEMS/POSSIBLE VIOLATIONS AND CORRECTIVE ACTIONS 1. The reclamation plan do•=_s not adequately address protection of 60 Davs of the pipeline facility f:com slides or damage as a result of the Receipt mining and reclamation operation. Under the Construction PB or PV:PB Material Rules and Regu:Lations, areas outside of the affected TOPIC(S):SC land shall be protected from slides or damage occurring during the mining and reclama t:ion operation (C.R.S. 34-32.5-116 (4)(i)). CORRECTIVE ACTION: The operator will need t:o file a revision to the permit to adequately address stability of the pipeline corridor. The revision will need to bee filed within 60 days of receipt of this report, or included witYi corrective action #2 noted below. 2. A slurry wall was constructed around Pit #5 and #6 and does not 60 Davs of conform to the postminir.~.g land use. In addition, the operator will need to provide sign-off from the Office: of the State Engineer (OSE) or provide sufficient amount of reclamation bond. If the OSE signs off on the slurry wall, the need for drawings, as-built certifications or bond will not be required. The amendment will need to be filed within 60 days of receipt of this inspection report. 3. Leafy Spurge was noted o:n site, and is considered a noxious weed 60 Davs of in Weld County. Rule 3.1.10(6) requires methods of weed control Receipt be employed for all noxious weed species.
