• <br />MINE ID # OR PROSPECTING ID # <br />INSPECTION DATE 11/10/94 <br />(Page 2) • <br />M94-005 <br />INSPECTOR'S INITIALS RCO <br />This inspection was performed by the Division to monitor the existing permitted operation and <br />to assess the Bite and plans involved with the proposed expansion (Amendment 001). Paul <br />McGehee was contacted about the inspection and met this inspector at the site. The amendment <br />application was briefly reviewed in the office. This inspector and Mr. McGehee were then <br />accompanied by the Bite manager, Mr. Mayger, during the inspection of the several phases of <br />the operation. <br />APPLICATION REVIEW <br />~.J This will be covered in detail in the adequacy review letter. Some of the main questions <br />concerning the application materials were as follows: <br />~ The use of a new haul road on the southeast side of the expansion area. The road should be <br />included in the permit area. Also ensure that if no cottonwoods, etc., are to be affected <br />by the road, that it is not depicted to pass through them. <br />~~, The existing topsoil berm (plus trees to be planted) along the north aide are not shown on <br />any map. <br />Be sure that all et ructuree within 200 feet of the boundary are shown on a map. Private <br />-' dwellings were observed near the northeast and southeast corners of the expansion area, and <br />possibly within 200 feet. <br />~, The applicant has an engineering study stating that disturbance may occur as close ae 30 feet <br />to a structure with no negative impact. If disturbance is planned to occur at distances less <br />than 200 feet to any structure, the structure's owner must provide a statement regarding <br />liability for damage. <br />~+ There is a corner of Phase II shown to be outside of the permit area on the map. The <br />boundaries of the phases and permit area should be clarified. <br />SITE INSPECTION <br />The weather was calm, with no dust blowing during the inspection. Within the existing (Phase <br />I) area, the crusher had been moved offsite, and the noise level of its operation was <br />therefore unable to be assessed. No offsite disturbance was observed during the inspection. <br />The topsoil stockpiles were in place on the west and north sides of the Phase I area. They <br />are not presently graded or seeded, but seem to be stable for the interim. They have been <br />left until fall seeding in a rough and "chunky" state. The operator explained how the pileai <br />surfaces will be smoothed and seeded at a date later this month (as recommended by the SCS). <br />The operator will please notify the Division as to the timing of the grading, seeding and <br />planting of the berms. <br />The access road along the north was watered. An overhead water tank for the spray truck is <br />kept permanently full. Water for the tank is pumped from the ditch which passes on the east <br />side of Phase I. The ditch, which is planned to be rerouted by piping around the west side <br />of Phase I, was currently flowing. The required permit sign and markers were observed. <br />There were about 10 acres now mined. Depth was about 20~feet, and aide elopes were about <br />3:1. Stockpiles of primarily gravel covered about 10 acres. Piles reached a height of 30 <br />feet. <br />A wet area of about 5 acres was seen in the northeast portion of the expansion area (Phase <br />III) and in the adjacent off-permit area, down on the bottomland. The site seemed to be a <br />flooded pasture or hay field, and not a perennial wetland, judging by the vegetation. <br />However, these areas can be sensitive, and thorough assessment might not be possible in one <br />inspection. If operations are proposed to occur there or nearby, the operator should provide <br />information as to source of water on that site, and seasonality and nature of surface and/or <br />groundwater at the site. The area should be adequately represented on the map also. <br />A pond structure excavated in the southeast corner of Phase I is present, though not used, <br />because of the tank mentioned above. It is stable, and will remain until mining reaches that <br />corner. <br />
