., ,~ <br />(Page 41 <br />• <br />MINE ID # OR PROSPECTING ID # M-83-758 <br />INSPECTION DATE 2/27/97 INSPECTOR'S INITIALS WHE <br />The following problems IPBI and/or possible violations IPVI land suggested corrective actions) were identified during this inspection. <br />The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted <br />on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1 . Mine identification sign was posted at the ranch entrance and did not display all information required by <br />Rule 3.1 .12111. Affected area perimeter markers, required by Rule 3.1.1212) were not observed. <br />CORRECTIVE ACTION: Post and maintain affected area perimeter markers, delineating the permit area, <br />as required by 3.1 .1 2121• Ensure mine identification sign is posted at the entrance to the mine site and <br />displays the name of the operator, the permit number and a statement that a reclamation permit has been <br />issued by the Colorado Mined Land Reclamation Board, as required by 2.1.1211 I. <br />3/31 /97 <br />PB or PV PB <br />TOPICIS) SM <br />2. Original permit was approved for 9.0 acres affected area. Annual reports for 1 995 and 1 996 indicate <br />9.85 acres affected area. SO-01 suggests that operator may desire to modify configuration of permit <br />area. <br />CORRECTIVE ACTION: If the operator does not desire to modify the permit area from the originally <br />approved 9.0 acres, please submit corrected map, showing 9.0 acres rather than the 9.85 acres indicated <br />on annual report maps. Additionally, please submit a corrected annual report for 1996, showing 9.0 acres <br />under item 1 a. of the annual report form. If operator desires to modify the permit acreage andlor <br />configuration of permit boundary, please submit such request under the Technical Revision or Amendment <br />process described within the Rules. <br />4/30/97 <br />PB or PV PB <br />TOPICISI AR <br />3. DMG records indicate that financial warranty is scheduled for review.
