• (Page 21 <br />• <br />MINE ID # OR PROSPECTING ID # M-1998-022 <br />INSPECTION DATE 10-19-2000 INSPECTOR'S INITIALS SSS <br />OBSERVATIONS <br />This was a routine monitoring inspection conducted as part of DMG's ongoing monitoring program for mine sites in <br />Colorado. <br />Mining has created a staging area and mining access bench. The bench was created by filling swales with undesirable rock <br />and by cut and fill manipulation of some earth material underlying the surface rock. The staging area and mine bench are <br />larger than approved in the permit. The staging area is estimated to be 100 ft. by 100 ft. rather than the permitted 50 ft. <br />by 50 ft. The mine bench is estimated at 300 ft. by 50 ft. rather than the approved 100 ft. by 20 ft. Additionally, the <br />staging area has a near vertical highwall, approximately 100 ft. long by 20 ft. high, where material was cut from the slope <br />to create the staging pad floor. This permit was approved with the stipulation that all slopes would be maintained at a <br />grade of 2:1 or less. <br />Paul Rodriguez (Gallegos Corp.) indicated that the proposed staging area and mine bench were to confining for efficient <br />operation of the site. He indicated that a revision to the permit would be submitted to change the dimensions of these <br />features. Because these features are larger than approved in the mine plan, the site is currently underbonded. Gallegos <br />Corp. must address these problems through submittal of a revision application, received by DMG no later than 11-30-2000. <br />The reclamation cost estimate will be recalculated upon approval of the revision application <br />The oversteepened highwall must be reduced to a 2:1 grade by no later than 11-30-2000. The operator is reminded that <br />all slopes must be continually maintained at no steeper than a 2:1 grade. Failure to adhere to this stipulation may result <br />in future enforcement actions. This stipulation was placed on the permit due to slope stability concerns expressed by <br />adjacent landowners, and because the permit lacks a slope stability demonstration from a professional engineer. <br />Several of the power poles for the on-site powerline are not indicated on the permit maps. Please ensure the maps provided <br />with the revision described above also include the accurate location of all the on-site power poles. <br />The operation has exposed some earth material that has been used to create the staging area and mining area benches. <br />The operator will need to address installation of the stormwater control ditch at this time. The benched surfaces of the <br />permit area and the staging area highwall contain enough soil that surface runoff will most likely contain sediment that <br />could be discharged off-site. The stormwater control features described in the stormwater control permit must be installed <br />no later than 11-30-2000 to protect the hydrologic balance of the area and prevent off-site discharge of sediment. No <br />evidence of previous sediment discharge from the site was noted during this inspection. <br />No reclamation has yet been conducted. The revision described above will need to adjust the areal figures specified for <br />the active mining area. The revision should also address when reclamation will begin on mined out areas. <br />Responses to this inspection report should be directed to Steve Shuey at the Division's Durango Field Office.
