(Page 4) <br />MINE ID # OR PROSPECTING ID # M-1980-003 <br />INSPECTION DATE10-29-2002 <br />INSPECTOR'S INITIALS SSS <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this <br />inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible <br />violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation <br />Board. The inspector noted on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />1 • The mine identification c_ign wac_ observed but was not in comolian ... Some aff .t -d ar a bo mda t <br />markers were in place but not enough to make the permit boundary readily identifiable. <br />CORRECTIVE ACTION: <br />Please install a mine identification sign and adequate boundary markers per the requirements of <br />Rule 3.1.12 and submit photographic documentation of compliance. Also, submits technical revision <br />to the permit that includes new permit maps that clarify the permit boundary location and include all <br />mining related features associated with this operation by the correction date noted. <br />2. Noxious and/or nmblematic weed infestation was observed within the affected area. <br />CORRECTIVE ACTION: <br />A weed control plan developed in consultation with. and approved bv. the appropriate local <br />weed control authority must be submitted as a technical revision to this permit by the <br />correction date noted. <br />3. A 55 gallon drum and a couple of 5 gallon buckets reportedly containing hazardous materials <br />were not stored within adequate spill containment. <br />CORRECTIVE ACTION: <br />Ensure these materials are stored and maintained within adequate spill containment so these <br />materials are confined and easily cleaned up in the event of a spill.
