INSPECTION REPORT 6 <br />Gulch #'s 1,2,3,4,5,6 certifications were noted but did not record them <br />in field notes. <br />There was a problem with locating designs and as built designs for <br />sediment ponds with drop inlet structures. Mr. Agnew was unable to find <br />them prior to the closeout so he was asked to forward via fax a copy of <br />one of the as builts designs for No Name #4. The data was found and <br />sent to the N,LRD then passed on from there. The reason for reviewing <br />the design initially was to get an idea for what kind of footer and how <br />much was put in for the drop inlets. The operator has had problems with <br />risers on some of the ponds heaving out from freeze-thaw cycles which <br />has resulted in embankment failure on one of their ponds. They have now <br />changed to open channel spillways on new construction. After reviewing <br />the as built, it is obvious why the riser heaves, there is no footer! <br />General haul road certifications were dated 3\20\90 and 2\7\86. <br />
