(Page 4) <br />MINE ZD # OR PROSPECTING ID # M-HO-244 <br />INS P]:CTZON DATE 11/2/94 INSPECTOR'S INITIALS JAP, MBL & WY-F <br />(1) The drain cover material placed during the period cited as a possible violation by <br />Gary Curtiss should be located and examined through appropriate excavations to ensure that <br />it does no~~ represent an anomalous concentration of oversize material. <br />(2) The drain cover material placed in the "road" pile near the top of the southeast <br />sector of heap leach pad should be examined to determine whether it represents an <br />anomalous concentration of oversize material. This should include excavation into the <br />body of the pile to examine it in three dimensions. <br />(3) The size gradation of the drain cover material in the pad #2 source area should be <br />recharacterized. Specific attention should be focused upon characterizing the apparent <br />variation between the differing materials observed in the excavated face. <br />(4) A plan with which to control the excavation of pad #2 drain cover material should be <br />developed for approval.
