He radioed the appropriate
personnel -~o commence an inquiry into whether the location could be reestablished.
Pad #2 - Drain Cover Material Source
We next traveled to pad #2 to examine the area from which the drain cover material is
excavated. The upper lift of pad #2 spent ore is being excavated, placed into trucks and
hauled to heap leach pad #1, where it is placed onto the geomembrane liner to serve as
protective drain cover material. An approximately 250 foot long cut face ie exposed. we
examined t:he face and discerned one central portion of the exposed face appeared to
contain a concentration of oversize material. The remainder of the material appeared to
contain less oversize. Jim suggested that it would be appropriate recharacterize the
source materials in order to quantify the apparent variance by zone. A loader merely
excavates the crushed material and places it into haul trucks. No selective control has
been exercised to avoid zones containing concentrations of oversized material. Jim
Pendleton expressed his opinion that QA control by an appropriately qualified professional
might significantly reduce the oversize material being delivered to the pad and thereby
decrease the required selective manual gleaning. If necessary, the rejected material
could be s~:reened to remove the oversize material.
Upper Are qua Gulch - Highway 67 Relocation Footprint
We next drove to the area of upper Arequa Gulch above the up gradient boundary of heap
leach pad #1. A berm is being placed in this area to carry the relocated Highway 67
across Arequa Gulch. The three underdrain pipes and their bedding backfill are being
extended beneath this berm footprint. The three pipes and their backfill had been
completed. It remains to pour a thrust block foundation upon which a riser will be placed
to extend the solid 24" underdrain pipe vertically to the surface drainage sump up
gradient from the relocated highway. The 24" perforated pipeline is capped at this
location. And the 4" solid pipeline ie temporarily capped awaiting extension to the
spring box which will be constructed below the Arequa Gulch spring when it is excavated.
The Upland Diversion Ditch Culvert
We drove a location just south of the crushers where we observed the excavation for the
culvert which carries the upland diversion beneath the crusher pad and the conveyors. The
upland div~=rsion ditch is largely excavated to rough grade. The culvert beneath Highway
67 remains to be placed.
The Crusher Truck Turnaround Heach
CCaVG had been cited earlier for a regarding topsoil salvage at the toe of the crusher
truck turnaround berm. General excavation fill being placed to construct the truck
turnaround berm east of the crushers had cascaded down onto and covered topsoil at the toe
of the berm. John Hardaway stated that the contractor had since excavated and recovered
as much of the topsoil as possible. We examined the area in question and agreed with
John's ass~artion.
Close-Out Conference
During our close-out conference we discussed resolution of our concerns regarding the size
gradation of the drain cover material. With John's participation we defined the following
topics of concern. We anticipate CC6VG will address these topics within the next several
days.
