The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS ' <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. Approximately 1000 gallons of dilute cyanide solution As soon as <br />breached the lined containment area at Pad 3 on July 6, possible <br />1995. The solution had seeped into a dry drainage PB or PV: PV <br />within the permitted area. The solution was TOPIC(S) AT, MP <br />neutralized with calcium hypochlorite. <br />CC&V will submit to the Division a brief report of all <br />information concerning the spill by 5:00 P.M. July 7, <br />1995. CC&V will collect a composite soil sample from <br />the gulch and will perform additional remediation if <br />necessary.
