ti •
~ MINE ID ~ OR PROSPECTING ID ~
INSPECTION DATE July 7, 1995
(Page 2)
M-80-244

INSPECTOR'S INITIALS TAS
1. A brief inspection was conducted of the Cresson Project - Phase I pad construction. The
clay underliner was being compacted in the upper reaches of Arequa Gulch. Synthetic liner
was being deployed in the aide slopes of the gulch. No concerns were noted.
2. Cripple Creek & Victor Gold (CC&V) noted that they had a spill of dilute cyanide solution
the previous day and that all proper authorities had been notified. The remainder of the
inspection focused on the details concerning the spill.
3. Approximately 1000 gallons of very dilute cyanide solution had overflowed the lined
containment area at Pad 3 for approximately 15 minutes on July 6, 1995. The solution flowed
downgradient along a county road and into a dry gulch for a total travel length of 1000 feet.
The spilled solution had apparently remained within the permitted area. CC&V had promptly
applied a treatment of calcium hypochlorite to the cyanide solution as it was seeping into
the alluvium. The cause of the spill was a break in a pipeline weld.
4. CC6V agreed to submit to the Division by 5:00 P.H. on July 7, 1995 a brief report
describing the corrective actions already applied. CC&V also commited to submittal of a
detailed report which includes the facts surrounding the spill, pending soil results,
laboratory analysis of the spilled solution, and a map showing the location of the spilled
solution. The detailed report will be submitted as soon as possible.
5. cC&V was informed that the Division will bring this matter before the Mine Land
Reclamation Board for review of a possible violation for failure to contain the cyanide
solution.
I & E Contact Addreee
NAHE John Hardaway
OPERATOR Crioole Creek & Victor Gold
STREET P.O.
