• (Page 41 • <br />MINE ID N OR PROSPECTING ID rr M-78-091 UG <br />INSPECTION DATE 7 2/ 1 /99 <br />INSPECTOR'S INITIALS RCO <br />The following problems IPB) andlor possible violations (PV) land suggested corrective actions) were identified during this <br />inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible <br />violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation <br />Board. The inspector noted on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />Drainage from a portion of the top of the pad has been deliberately directed off the <br />edge of the pad and down the waste slope, damaging the contour terraces and <br />gullying the slope face. This is not in compliance with the EPP. <br />CORRECTIVE ACTION: <br />The operator must perform the earthwork needed to prevent further drainage <br />originating on the pad from flowing down the dump face and repair the terraces. The <br />condition of these surfaces must be brought into conformance with the specifics in <br />the EPP. Such work is to be completed AND WRITTEN EVIDENCE OF SUCH <br />COMPLETION TO BE SUBMITTED TO THIS OFFICE by the date shown at right. <br />2. The drainage path of Deadman Gulch has been blocked by waste material placed by <br />the operator. Such drainage is not to be interrupted, nor is such material to be placed <br />in the drainage path. This is not in complaince with the approved mining plan. <br />CORRECTIVE ACTION: <br />The operator must remove such material from the gulch, and place it in a stable <br />location on the pad. If placement of fill material near the gulch is planned, only <br />benign fill material may contact the native gulch material, it may not expand the <br />"footprint" of the pad, and it must not contribute to potential sediment loading. Such <br />removal must be completed AND WRITTEN EVIDENCE OF SUCH COMPLETION TO <br />BE SUBMITTED TO THIS OFFICE by the date shown at right. <br />CORRECTION <br />DATE <br />9!15/99 <br />PB ar PV: PB <br />TOPICISI: SC, <br />HB <br />9/15/99 <br />PB or PV: PB <br />TOPICISI: H61 <br />MP . As a DMO, the operator must ensure that written follow up or TOPICISI: AR , _, _ <br />progress of operations are not overlooked. <br />CORRECTIVE ACTION: <br />The operator must provide written evidence of the correction of the two problems <br />described above, by the date shown for those two problems. Additionally, the <br />operator must provide brief but current operation information on the next annual <br />report. The corrective action date shown at right is the anniversary date, which is <br />when the annual report is due. <br />
