<br />~ MINE ID # OR PROSPECTING ID # M-B9-030 <br />INSPECTION DATE 10/09/97 <br />INSPECTOR'S INITIALS RCO <br />The following problems (PB) and/or possible violations (PV) (and suggested <br />corrective actions) were identified during this inspection. The problems <br />should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given <br />to reduce their severity when considered by the Mined Land Reclamation Board. <br />The inspector noted on the previous page should be notified of all corrective <br />actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />i~ <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. The site appeared to possibly have some offsite 11/25/97 <br />damage, though it was uncertain due to the boundary <br />marker having been knocked down. PB or PV:PB <br />TOPIC(S):OD <br />The operator must reinstall the marker at the correct <br />location, determine whether any offsite disturbance <br />had occurred, and if so, report it to this office by <br />the d,ste shown. (The date is the present decision <br />date for the conversion application.) The report must <br />include a description of the type and extent of <br />damag~=_, and a map. If there is no offsite <br />disturbance, submit such a statement. <br />If there is offsite damage, the <br />commit to either beginning full <br />offsite damage, OR proceed with <br />appli~~ation, to the point of ob <br />if such conversion will include <br />within its new boundaries. <br />operator must also <br />reclamation of the <br />the 112 conversion <br />wining the 112 permit, <br />this offsite area <br />2. Some Of the topsoil at the site was used to construct 11/25/97 <br />a ramp for mine vehicles, when such a use of the soil <br />is contrary to soil protection practices. PB or PV:PB <br />TOPIC(S):TS <br />The operator must cease using the soil in this manner, <br />and either stockpile it in a protected location away <br />from =urther disturbance or replace it on areas being <br />reclaimed. The operator must provide a statement to <br />this office by the date shown describing what was <br />performed, and a photo showing such activity. <br />(Page 4) <br />3.
