(Page 3) <br />MINE ID # OR PROSPECTING ID # M-1987-092 <br />INSPECTION DATE 08/31/06 INSPECTOR'S INITIALS ESC <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLSNS/POSSIBLB VIOLATIONS <br />AND <br />1' PROBLEM: <br />Permit boundary markers could not be located on site as <br />required by Rule 3.1.12 of the Construction Materials Rules and <br />Regulations. <br />CORRECTIVE ACTION: <br />The operator will need to poet permit boundary markers on site <br />and submit documentation in the form of photographs on or <br />before December 1, 2006. <br />2•/ PROBLEM: <br />A small oil/petroleum spill was noted on the road at the <br />entrance to the upper pit. The oil/petroleum spill is <br />approximately 5 feet by 3 feet. <br />CORRECTIVE ACTION: <br />The operator will need to excavate the contaminated area and <br />properly dispose of the material by an approved method (such as <br />send to an approved landfill, land farming, recycling, etc.) <br />and submit documentation of compliance (photos or receipts of <br />work completed) on or before December 1, 2006. <br />DATE <br />PB or PV:PH <br />TOPIC(S):SN <br />12/1/06 <br />PB or PV:PB <br />TOPIC(S):NP <br />12/1/06 <br />
