<br />~~ <br />(Page 4) • <br />MINE ID # OR PROSPECTING ID # M-79-781 <br />INSPECTION DATE 9l2/9S INSPECTOR'S INITIALS <br />The following problems IPBI and/or possible violations (PV) land suggested corrective actions) were identified during this inspection. <br />The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted <br />on the previous page should ba notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />Problems and their corrective actions which remain from previous inspections have been modified as follows: <br />1. Previous maps submitted by the operator indicate approximately 2 acres affected area beyond the <br />approved 2 acre boundary, do not clearly delineate the permit boundary, and do not satisfy the map <br />requirements of Rule 6.2.112), 6.3.3 and 6.3.5. <br />CORRECTIVE ACTION: Within thirty 130) days, by October 8, 1999, submit a mine plan map, which satisfies <br />the requirements of the Act and Rules. 10 18 / 99 <br />PV or PB PB <br />TOPICS AR. Description for how affected areas will be stabilized to control erosion and how operations will be <br />conducted to ensure that disturbances to the hydrologic balance will be minimized, appear absent from permit <br />documentation. The operator is required to satisfy the performance standards of Rule 3.1.5(2), (3), 15), 110), <br />(11), 3.1.6(1) and (3). Additionally, Rule 6.3.3(1) and 6.3.4(1) require that the permit demonstrate <br />compliance with the performance standards of Rule 3. DMG observations from previous inspections indicate <br />possible compliance problems with the above referenced Rules. <br />CORRECTIVE ACTION: Within thirty (30) days, by October 8, 1999, submit a Technical Revision to the <br />permit containing plans for stabilizing affected areas and controlling erosion, consolidation and containment <br />of potentially acid forming ore and waste rock materials handled since permit issuance, and demonstrating <br />how operations will be conducted to minimize disturbances to the hydrologic balance. 10 / 8 199 <br />PV or PB <br />TOPICS AR.
