<br /> <br />MINE ID # OR PROSPECTING ID # M-90-057 <br />INSPECTION DATE 10/26/99 <br />(Page 5) <br /> <br />INSPECTOR'S INITIALS ESC <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />DATE <br />1. The permit makes note of an exit shaft, but the maps do not pg or PV:PB <br />include the location of the Hunter shaft. The shaft disturbance TOPIC (S):AR <br />appears to fall within the 1.9 acres of disturbance stated for 60-Davs <br />the mine area, but not within the permit boundaries. <br />CORRECTIVE ACTION: <br />The operator will need to clarify the permit to include the <br />affected area of the Hunter. The permit text and maps must be <br />submitted to the Division within 60-days of the receipt of this <br />report. <br />2. Permit boundary markers could not be located at the mine site as <br />required by Rule 3.1.12. PB or PV:PB <br />CORRECTIVE ACTION: TOPIC(S):SL2 <br />The operator will need to post the boundary markers and submit 60-Davs <br />to the Division photographs that the markers were posted within <br />60 days of this inspection report. <br />3. A number of trees are growing on the tailings embankment that <br />could jeopardize the integrity of the structure. PB or PV:PB <br />CORRECTIVE ACTION: TOPIC(S):MP <br />All trees will need to be removed from the tailings pond 60-Days <br />embankment and the operator shall submit proof in the form of <br />photographs within 60-days of receipt of this report. <br />4. Temporary ore piles were stored at the mine site and the mill <br />site, but were not identified in the application. <br />CORRECTIVE ACTION: PB or PV:PB <br />The operator will need to identify areas where temporary ore TOPIC (S):MP <br />will be stored, and how the area will be lined to prevent the 60-Davs <br />contamination of surface water and groundwater. Location and <br />description of the temporary ore stockpiles must be submitted to <br />the Division in the form of a Technical Revision within 60-days <br />of the receipt of this report. <br />5. On June 5, 1990, the Division approved the Hopemore & Leadville <br />Mine with four (4) conditions to the permit. The Division has not <br />received any of the information required by the conditions. <br />CORRECTIVE ACTION: PB or PV:PB <br />The operator will need to inform the Division the status of each TOPIC (S):ST <br />condition noted on the approval letter, and submit all relevant 30-Davs <br />documentation within 30-days of receipt of this report. <br />
