(Page 3) <br />MINE ID # OR PROSPECTING ID # M-82-015 <br />INSPECTION DATs: 11/13/98 INSPECTOR'S INITIALS CLK <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTION <br />DATE <br />A stockpile of concrete is being stored on the site. This stockpile, and plans for Dec. 28, 1998 <br />reclaiming it, are no[ included in the current reclamation plan or financial PB or PV PB <br />warranty. This is in violation of Rule 6.4.5(2). CORRECTIVE ACTION: The TOPIC(S) FN <br />Operator must submit either a Technical Revision to the permit for reclamation <br />actions to be taken regarding the stockpile (such as grading, topsoiling, and <br />seeding of the stockpile); or submit a letter from the Landowner, stating the <br />Landowner's wish that the stockpile be left in place after release of the site for use <br />as flood control material, within 30 days.. <br />
