Table 5 - CQA Earthworks Testing Summary – Soil Liner Fill: The first nine samples of Soil <br />Liner Fill were taken in 2023, while the rest were taken in 2025. Please clarify why some samples <br />were taken two years prior. <br /> <br />R/. The SLF utilized for the Stage B.1 construction was screened by JHL in 2023. NewFields <br />Personnel and NewFields Laboratory were onsite during the screening of SLF in 2023. NewFields <br />obtained samples during screening activities to certify that the screened material met or exceeded <br />specifications. The Laboratory and Personnel was demobilized from site before the entire stockpile <br />could be certified in December of 2023. Testing of the stockpile resumed in April of 2025, ensuring <br />testing frequencies were met or exceeded for Stage B.1. <br /> <br /> <br />12. Table 6 - CQA Earthworks Testing Summary – Drain Cover Fill (Crushed Ore): The <br />Specification – Percent Passing column header should list the range for the 2” and 0.75” grain <br />sizes identified in the specification and as shown on Appendix G.3 data sheets. Please correct the <br />table. <br /> <br />R/. Table 6 has been corrected and provided as part of the amended report. <br /> <br />Appendices <br /> <br />13. Appendix A - Staff Schedule for CQA Monitor Personnel: On Page 1, Angel Villar was present <br />on certain dates as “LS” (lead soils technician) but fails to include this acronym on the Page 1 <br />table foot note. Please amend the footnote to include this title. <br /> <br />R/. Appendix A has been corrected and provided as part of the amended report. <br /> <br /> <br />14. Appendix A - Staff Schedule for CQA Monitor Personnel: The deputy project certifying <br />engineer (DPCE), Scott Parker, is incorrectly labeled as the “SPCE” in the table on Page 1. Please <br />either justify this discrepancy or correct the table to reflect the correct title. <br /> <br />R/. Appendix A has been corrected and provided as part of the amended report. <br /> <br /> <br />15. Appendix D - Soil Liner Fill Acceptance Forms: These SLF Acceptance Forms show sequential <br />area locations and size (e.g. 6/13/25: P-4962/4966 - 45,023 sq.ft.) however, the 7/9/2025 form has <br />only the starting panel listed and no square footage amount. Please clarify the reason for the <br />missing data, confirm that samples were included in this missing area, and revise the acceptance <br />form accordingly. <br /> <br />R/. The missing information was an oversight from the Lead Geosynthetics Technician. The original <br />Acceptance Form had signatures from Ben Melly (Lead Geosynthetics Technician), James Land (JHL <br />Superintendent), Daniel Egley (SSR Construction Supervisor), and Jose Quiroz (EC Applications <br />Superintendent). The corrected form has signatures from Tyler Wendlandt (Project Resident), Vittorio <br />Mazzarella (JHL Project Manager), Zac Cook (SSR Construction Supervisor) and Jose Quiroz. All <br />signees on the corrected form were onsite on July 9th and are the direct supervisor or peer of the <br />original signees. The form was dated to the day of approval for continuity purposes, as has been done <br />historically with NewFields acceptance form corrections. The additional signed form has been added to <br />Appendix D. The updated Appendix D and provided as part of the amended report. <br /> <br /> <br />16. Appendix D - Soil Liner Fill Acceptance Forms: The square footage measurements given on all <br />SLF acceptance forms add up to ~1,119,668 sq. ft.
