PERMIT#:M-2002-118 <br /> INSPECTOR'S INITIALS:AMG <br /> INSPECTION DATE: September 10,2025 <br /> OBSERVATIONS <br /> The Velarde Pit was inspected by Amber Gibson with the Division of Reclamation,Mining and Safety <br /> (Division/DRMS). The inspection was completed as part of the Division's routine monitoring inspection <br /> program. The site was previously inspected by the Division on October 12, 2021 as part of a Successor of <br /> Operators revision. Sara Weimer and Kurt Wasinger(representing the Permittee/Operator—Oldcastle SW <br /> Group, Inc.), accompanied me during the inspection. The sky was partly cloudy, and the weather was hot. <br /> The Velarde Pit is located in Huerfano County approximately 13 miles northwest of Walsenburg, Colorado. The <br /> current entrance to the pit is located off the southwest side of County Road 613. The site is permitted as a 300- <br /> acre 112c Regular Operation Construction Materials Permit, where 149 acres are approved to be affected, and <br /> 30 acres are approved to be affected at any-one-time. The primary commodity mined at the site is sand and <br /> gravel. The approved post-mining land use is rangeland. <br /> Availability of Records: <br /> The annual fee,map, and report are current and paid through June 9, 2026. <br /> Revision History <br /> • 2005: (SO1) Succession of Operators. DG Huskin Construction Co transferred to APC Southern <br /> Construction. <br /> • 2008: (TR1) Removal of acres from Phase 3, and adding acreage to Phase 1. <br /> o Note* This revision application was denied on the grounds that acres not previously approved to <br /> be affected cannot be added through a Technical Revision,nor can affected acreage be removed <br /> through a Technical Revision. <br /> • 2014: (SI1): Surety Increase as a result of a routine monitoring inspection discovering that <br /> approximately 30 acres had been affected, exceeding the 20-acre maximum disturbance approved at the <br /> time. <br /> • 2014: (TR2): Increased the maximum allowed disturbance at any-one-time from 20 to 30 acres. <br /> • 2019: (S12): Routine Surety Increase following a routine monitoring inspection. <br /> • 2021: (S02) Succession of Operators. APC Southern Construction transferred to current Permittee— <br /> Oldcastle SW Group, Inc. <br /> • 2021: (SI3): Routine Surety Increase,prompted by S02 to cover reclamation liability at the site. <br /> The Division found that the recent annual report maps were inadequate. This has been cited as a problem <br /> above. <br /> 1. For the 2026 submittal of the annual report map please refer to the Annual Report Form. The Annual <br /> Report Form states that as required by the Colorado Land Reclamation Act for the Extraction of <br /> Construction Materials (C.R.S. 34-32.5-116),the Permittee shall attach a map to the report that <br /> accurately depicts: <br /> i. the permit boundary, <br /> ii. the current affected area boundary and; <br /> iii. the location of the acreages specified in Items no. 8-12 and 15. <br /> Items 8-12 and 15 on the Annual Report Form are listed below. <br /> 98.Number of acres currently affected(mining+ incomplete and or unreleased reclamation). <br /> Page 3 of 18 <br />
