PERMIT#:M-2002-118 <br /> INSPECTOR'S INITIALS:AMG <br /> INSPECTION DATE: September 10,2025 <br /> INSPECTION TOPIC: General Compliance with the Mine Plan <br /> PROBLEM: The Operator has failed to comply with the approved mine plan,the Operator has disturbed more <br /> acres than the maximum allowed disturbance of 30 acres. The current mine plan shall be updated and clarified <br /> pursuant to C.R.S. 34-32.5-112 (1)(c)(VI). The Operator must provide sufficient information to describe or <br /> identify how the Operator intends to conduct the operation. <br /> CORRECTIVE ACTIONS: The Operator shall submit a Revision,with the revision fee, to update and clarify <br /> the current approved mine plan and Exhibit L Reclamation Cost Estimate to reflect existing and proposed <br /> activities as dictated by the results of an MLRB hearing by the corrective action date set in the Board's Order. <br /> CORRECTIVE ACTION DUE DATE: Tentative MLRB Hearing Dates—December 17— 18, 2025 <br /> INSPECTION TOPIC: Signs & Markers <br /> PROBLEM: The mine sign displays outdated information. The Operator shall, at the entrance of the mine site <br /> post a sign in compliance with Rule 3.1.12(1), which shall be clearly visible from the access road, with a <br /> minimum size equaling one hundred and eighty-seven (187) square inches, such as eleven (11) inches in height <br /> and seventeen (17)inches in width, with appropriate font size,with the following: the name of the Operator, a <br /> statement that a reclamation permit for the operation has been issued by the Colorado Mined Land Reclamation <br /> Board; and the permit number. <br /> Also,the majority of the affected area boundary markers are missing or incorrectly placed. This is a problem for <br /> failure to maintain boundary markers around the affected area as required by Section 3.1.12(2) of the Rule. <br /> CORRECTIVE ACTIONS: The Operator shall update and/or replace the mine sign to reflect the current <br /> Permittee, and conduct a survey and replace the boundary markers in the correct locations. The Operator shall <br /> provide proof to the Division that this has been done by the corrective action date. <br /> CORRECTIVE ACTION DUE DATE: 11/30/25 <br /> Page 2 of 18 <br />
