The Inspection was completed on August 13, 2025, by Clayton Wein. Elk Ridge <br />Mining and Reclamation was represented during the inspection by Mindy Sickels. The Weather was clear with a <br />temperature of 90℉. Ground conditions were dry at the time of the inspection. <br />AVAILABILITY OF RECORDS – Rule 5.02.4(1): <br />The records for the New Horizon North Mine are located at the mine office in Nucla, CO. The records were well <br />kept and up to date. Please see the Availability of Records Form attached to the end of this report for more details. <br /> <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br />Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br />Drainage – Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />Pond 001 was reduced in size to a stock pond at the landowner’s request. All the other sediment ponds have been <br />reclaimed at the New Horizon North Mine. Pond 001 was dry and stable during the inspection (Photo 1). The <br />embankments of the stock pond had minor rills. These rills appear to be stabilizing as the vegetation on the <br />embankments grows. The rills did not appear to be affecting the embankments stability. <br />The southern portion of the permit area, referred to as NHN-007, had a series of silt fences installed to catch any <br />runoff from the southern portion of Irrigated Pasture NHN-004. The Silt fences were in good condition and no <br />erosional features were identified. Vegetation has grown in where the water flows across NHN-007 to the south <br />and helped stabilize the dryland pasture reclamation. <br />Three groundwater well clusters are located within the New Horizon North Mine permit area. Wells GW-N56, <br />GW-N57 and GW-N58 are located within the southwest portion of the permit area. This cluster of wells was <br />observed to be in good condition. The wells were stable, and the caps were observed on the PVC pipes. The GW- <br />N53, GW-N54 and GW-N55 wells are located within the western portion of the permit boundary (Photo 2). These
