All corrections are to be initialed <br /> and dated by the individual making the error, if possible, or by the investigator. All <br /> paperwork completed in the course of collecting and shipping samples must be <br /> correct, accurate, and defensible in a court of law. <br /> The complete COC will accompany the sample from the site, through delivery to <br /> the contracted laboratory. To ensure complete documentation of sample custody, <br /> field personnel and laboratory personnel will sign and date the COC upon <br /> shipment and receipt. <br /> 9.6. Field Log Sheet <br /> The field log sheets contains the first record in the chain of custody of the sample. <br /> It is previously discussed in Section 4.0 (Field Data) of this document. <br /> 9.7. Chain of Custody Record <br /> 1111 A chain of custody record will be completed and must accompany each sample or <br /> each cooler of samples (see Appendix A). The record will include the following <br /> information: specific area/permit name, sample identification, sample type (well, <br /> grab, soil, other), preservative(s) used, whether the sample was filtered, type of <br /> analysis(es) required, number of sample containers, signatures of persons <br /> involved in the chain of possession, inclusive dates of possession, a unique NMC <br /> identification number which includes the area (NA = North Area, SA = South Area, <br /> RN = Rain Mine, EM = Emigrant Mine, WA = Waste, HO= Hollister) and date, cooler <br /> number, and method of shipment. If an analysis is to be rushed, state this on the <br /> Chain of Custody. One copy of the chain of custody is kept by Newmont. The <br /> laboratory keeps one copy for its records, and returns a copy to Newmont with <br /> the analysis reports. <br /> 9.8. Shipping Papers <br /> A shipping label is attached to the top of cooler or shipping container along with a <br /> laboratory's address label.
