Upon a review of the A004-A006 Table of <br /> • Historical Underground Workings, the Date Observed Visually for all features falls between <br /> 2006 and 2007, perhaps long before AM14 was envisioned which may have limited the <br /> scope of those previous surveys. Please discuss if a new survey of underground workings <br /> will be conducted. The use of older imagery and on-the-ground observation may be <br /> needed. <br /> Drawing A004-A006 have been provided for information purposes only. Prior to the commencement of <br /> any construction, CC&V and the contractor will review the underground working data currently available <br /> in CC&V's database and complete a field survey to verify all known underground working and if <br /> possible, identify unknown underground workings. Next, the contractor will complete a drilling campaign <br /> to verify the location and the limits of the identified underground workings which need to be remediation <br /> as part of the expansion. Then CC&V and the Engineer will develop remediation plans and document <br /> the construction of the remediation, which will be included in the Certification report. <br /> 52. Please call attention to (highlight, bold row border, create separate column, etc.)the <br /> Surface Disturbances located within the scope of AM14 on Drawings A004-A006 Table of <br /> Historical Underground Workings. <br /> Drawing A004-A006 have been provided for information purposes only. Please also make this revision to Figure 2 of the <br /> report. <br /> The contour on drawing A011 has been corrected. <br /> 54. Drawing A012 VLF2 Phase 4 Stage A Grading Plan shows a proposed leak detection <br /> trench and manhole which appears to be completely new. Please revise the drawing to <br /> include the existing location of SG LDS 3 and provide the plan for how SG LDS 3 will be <br /> extended to the edge of the expanded facility to continue functioning and be inspectable. <br /> Please make the same change to Drawing A013 VLF2 Phase 4 Stage B Grading Plan for SG <br /> LDS 4 and Drawing A014 VLF1 Phase 6 Stage A Grading Plan for LDS 12 and 13. <br /> The existing Leak Detection Manholes and Trenches are now included on Drawings A012, A013. <br /> The existing Leak Detection Manhole associated with SG LDS3, will be removed and relocated outside <br /> the VLF2 Phase 4 Footprint, near the proposed manhole associated with the VLF2 Phase 4 Leak <br /> Detection Trench. A solid 3" Diameter HDPE SDR 17 pipe with watertight couplers will be installed <br /> between the abandoned SG LDS 3 manhole and the proposed relocated manhole. It is envisioned this <br /> pipe will parallel the proposed VLF2 Phase 4 Stage A Leak Detection Trench. This work will be <br /> completed during the construction of VLF 2 Phase 4 Stage A. <br /> The existing Leak Detection Manhole associated with SG LDS 4 will be removed, and the perforated <br /> pipe from SG LDS 4 will extend and connect into proposed VLF 2 Phase 4 Stage A Leak Detection <br /> Trench. This work will be completed during the construction of VLF 2 Phase 4 Stage A. <br /> • The proposed Leak Detection Manholes final location will be determined during construction of the <br /> Leak Detection system and will not interfere with the existing Leak Detection Trench or Manhole. <br /> SSR Mining Inc.
