EXHIBIT C <br /> Rule 6.4.3 <br /> Pre-mining and Mining Plan Map(s) of Affected Lands <br /> As needed, one or more maps are attached as exhibits to legibly portray the following <br /> information: <br /> (a) all adjoining surface owners of record; <br /> i. See Exhibit C(a), consisting of a map showing all adjoining surface owners of record <br /> per the Weld County Assessor. <br /> (b) the name and location of all creeks, roads, buildings,oil and gas wells and lines,and <br /> power and communication lines on the area of affected land and within two hundred <br /> feet of all boundaries of such area; <br /> i. See Exhibit C(b)(1). There are no creeks within the extraction area of affected <br /> land or within 200 feet of the affected land. The closest creek to the affected land is Lost <br /> Creek which is south and east of the permit area. <br /> Well Permits: Permit No. 56043- (WDID n/a) — Permit expired 5/8/1974 (no well <br /> constructed); <br /> Seven temporary groundwater monitoring and observation holes were used to determine <br /> the depth of the groundwater within the permit area.A map showing the location of these <br /> monitoring holes is included as Exhibit C(b)(2). For additional water information, <br /> including the Piezometer Report,refer to Exhibits G,G-1,and G-2. <br /> ii. The only roads within 200 feet of the affected land are the private access road and a <br /> Dirt two-track trail along the eastern side of Section 27. Highway 34 is <br /> approximately 8.6 miles north of the permit area. See Exhibit C(g) and Exhibit <br /> C(h). <br /> iii. There are no buildings on the area of affected land or within 200 feet of the affected <br /> land. <br /> iv. There are no oil and gas wells or lines located on the area of affected land or within <br /> 200 feet of the affected land. The closest oil and gas tanks and piping are located <br /> approximately 660 feet to the west of the proposed permit area.
