Compliance With Mine Plan N - Support Facilities Not On-site <br /> N - Other N - Special Categories Of Mining <br /> N -Processing Waste R -Topsoil <br /> COMMENTS <br /> This was a partial inspection conducted by Lucas West of the Colorado Division of Reclamation, Mining and <br /> Safety's Active Mines Program. The site is in final reclamation however no reclamation activities were taking <br /> place at the time of the inspection. The weather was clear and dry. Contact was made with Mindy Sickles and <br /> Jason Carver,however they did not accompany the inspection as they were collection water samples at the time. <br /> Eight Photos accompany this report to illustrate the observed site conditions. <br /> The overall footprint of the site is in excellent condition. The Operator and it's contractors have done an <br /> exemplary job at conducting reclamation efforts. No maintenance items or problems were noted during this <br /> inspection. <br /> HYDROLOGIC BALANCE -Rule 4.05 <br /> Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br /> Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br /> Drainage—Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br /> During this inspection,the ponds in and around the backfill area were observed. The inspected ponds <br /> were Ponds 12, 13, 15 and 16. Pond 12 as well as its contributing ditch are in good condition,well vegetated and <br /> appeared to be stable at the time of the inspection. The ditch and pond contained no water and showed no <br /> evidence of recent flow or impoundment. Pond 12 can be seen in Photo One. Pond 13 was observed,with the <br /> typical amount of water as seen in Photo Two. The pond appeared stable,the banks are well vegetated, and the <br /> inflow and aeration device was actively filling the pond. The vegetation appears to be healthy and the aeration <br /> efforts appear to be working as the pond itself showed no visual signs of water quality impairment. The rock lined <br /> conveyance leading from the backfill area was also observed and in good condition. The structure shows no signs <br /> of recent flows, and no evidence of erosion or head cutting was noted. The conveyance can be seen in Photo <br /> Three. Photo Four shows the condition of Pond 15,which also contained no water. The contributing ditch to <br /> Pond 15 was also observed and was unobstructed and able to function as designed. Pond 16, located near the <br /> entrance to the backfill area was also dry,well vegetated and in good condition.
