Clyde & <br />Co., for the Rifle Gravel Pit #1, Permit M-2021-052. The following items are incomplete for the <br />application to be processed: <br />Application Form <br />1. The notary on pages 9 is incorrect. The current permittee is IHC Scott, Inc. <br />2. Incorrect application fee submitted. $115 was submitted which is the Hard Rock fee <br />amount. This is a Construction Materials site and the SO fee is $144. Please submit <br />the remaining $29. <br />3. Page 8, applicants’ agreement has the county location listed incorrectly. This <br />operation is located in Garfield County not Rio Blanco <br />4. The permit number is listed incorrectly; please update to M-2021-021. <br />Performance Warranty <br />5. Page 1 of the performance warranty signed by John Thompson has the county <br />location listed incorrectly. This operation is located in Garfield County not Rio <br />Blanco. <br />6. The permit number on page 1 is listed incorrectly, please update to M-2021-021. <br />7. The submitted Affidavit of Authority signed by Brandon Hale was a photocopy and <br />listed the permit number incorrectly. Please provide an original with the permit <br />updated.
