ATTACHMENT NO. 2 TO BID FORM <br /> LIST OF PROPOSED EQUIPMENT/LABOR RATES <br /> CONTRACTOR shall provide a list of the proposed equipment dedicated to completion of the <br /> work as specified in the contract documents with his bid and as provided below. Provide Unit <br /> Billing Rate for each piece of equipment and operator, including equipment that may not be <br /> proposed for the project but needed in the event additional or out of scope work needs to be <br /> billed on a Time and Materials (T&M) basis as agreed to by OWNER. Use a separate sheet of <br /> paper if necessary to include all potential equipment needed. Emphasis will be placed on the <br /> CONTRACTOR'S ability to complete the work in accordance with the requirements of the <br /> contract documents as quickly as possible.
