Epic <br /> Fh <br /> Construction& <br /> Environmental Inc. <br /> • Mobile equipment shall be equipped with roll-over-protection (ROPS) and/or falling object <br /> protection structures (FOPS) as required by MSHA regulations. <br /> • All machinery shall be secured to ensure zero mechanical potential prior to work being performed on <br /> the machinery. <br /> • Equipment shall be maintained in safe condition. <br /> HEALTH CONSIDERATIONS <br /> • Sanitary potable water shall be provided and maintained. <br /> ENVIRONMENTAL: LIQUID WASTES <br /> • All used fluids will be drained into clearly marked DOT containers. <br /> • Equipment fueling and service procedures will not be performed on topsoil. <br /> • EPA Prevention and Control Procedures are to be followed in the event of an accidental spill. <br /> • Reportable spills will be properly and sufficiently clean in accordance with State of Colorado And <br /> EPA regulations. <br /> ENVIRONMENTAL: SOLID, NON-HAZARDOUS WASTES <br /> • Construction debris and excess materials shall be disposed of properly. <br /> FIRE PROTECTION <br /> • Keep Fire extinguisher within working area at all times <br /> • Keep shove in working area <br /> • Specific procedures to ensure that fire hazards are addressed and controlled. <br /> o Call 911 for help <br /> ■ Use fire extinguisher only while safe <br /> • PASS <br /> o Pull Pin—to activate extinguisher <br /> o Aim—at the base of the fire <br /> o Squeeze—&release handle in short burst. <br /> o Sweep—across the base of the fire <br /> o Evacuate when necessary or unsafe! <br /> • Use, storage and segregation of flammable and combustible materials must be in accordance with <br /> NFPA regulations. <br /> • Contractor will immediately notify safety project manager of any fire emergency. <br /> • Fire extinguishers must be provided for all mobile equipment and at all areas where hot work is being <br /> performed. <br /> • Fire extinguishers must be maintained and inspected in accordance with <br /> • MSHA/OSHA regulations on/in all mobile equipment. <br /> WEATHER MONITORING <br /> • In case of heavy rain the site shall be evacuated for personnel protection. <br /> Contractor Representative: <br /> Signature Date <br /> Employee: <br /> Signature N <br />
