Epic <br /> Fh <br /> Construction& <br /> Environmental Inc. <br /> Safety, Health & Environmental Action Plan <br /> SITE ACCESS <br /> • No person may enter the job site without first receiving hazard training/site specific training from <br /> site supervisor. <br /> o All Employees must have HAZWPER training <br /> • Duncan Smith is the safety representative. <br /> • Employees are required to travel to and from their work areas by designated routes. <br /> • All equipment is will undergo daily walk around inspection to ensure safety measures are in place. <br /> • No persons shall enter an area that presents a fall risk without having donned proper fall protection <br /> equipment. <br /> PART 48 TRAINING REQUIREMENTS <br /> • Each employee shall receive site specific training,comprised of all aspects of the contract's Safety, <br /> Health and Environmental Action Plan (SHEAP). <br /> SUBSTANCE ABUSE SCREENING <br /> • Alcohol and drugs are not allowed on the job site. <br /> PROACTIVE SAFETY <br /> • Duncan Smith is the designate person to be the safety representative responsible for safety and <br /> health at the work site. <br /> • The safety meetings are conducted on a weekly basis. <br /> o All safety meetings shall be documented and include the date,subject matter,trainer and all <br /> participants signatures. <br /> • Task training shall be given prior to conduction new jobs;training will be conducted and signed off <br /> on by a competent person. <br /> GENERAL SAFETY STANDARDS <br /> • Horseplay will not be tolerated. <br /> ACCIDENT REPORTING <br /> • All accidents and injuries must be immediately reported to the project manager via the accident <br /> report form. <br /> o All reportable accidents shall be investigate all such occurrences and develop an action plan <br /> to prevent their recurrence. <br /> FIRST AID <br /> • First aid kits meeting MSHA (CFR 75.1713-7 or 77.1707) requirements shall be maintained in a <br /> sanitary condition and placed at all work sites. <br /> EQUIPMENT OPERATION <br /> • Seatbelts shall be used at all times. <br /> • Equipment always has the right-of-way. <br /> • Safe parking procedures (chock blocks,etc.) shall be used when necessary. <br /> • All lights,horns,brakes,wipers,back-up alarms and other safety equipment shall be fully operational <br /> at all times. <br /> • Each piece of equipment shall have at least one portable fire extinguisher. <br /> • All mobile equipment,with the exception of pickup trucks with unrestricted rear view,will be �I <br /> equipped with back-up alarms. <br />
