GENERAL BID SPECIFICATIONS <br /> The Colorado Inactive Mine Reclamation Program, General Bid Specifications 2009 which include <br /> General Conditions of the Contract, Standard Work Specifications, and Standard Drawings and <br /> Figures are intended to complement these Special Conditions. Please refer to the Standard Work <br /> Specifications for all applicable types of work required in the Special Conditions. The General <br /> Conditions of the Contract apply to all work covered in the Special Conditions. <br /> PROJECT DATES <br /> The project is scheduled for construction between September 11, 2023 and October 6, 2023. <br /> This includes ten (10) days mobilization time. These dates are tentative and may change. <br /> TIME OF COMPLETION <br /> The time of completion allowed for the project is twenty-six (26) calendar days after receipt of the <br /> Notice to Proceed. This includes ten (10) days mobilization time. <br /> NOTICE OF AWARD AND EXECUTION OF DOCUMENTS <br /> A Notice of Award will be sent to the apparent low bidder within a reasonable time following the <br /> bid submission deadline. The Division of Reclamation, Mining and Safety must receive the <br /> following by the date specified in the award letter: <br /> (a) Certificates of Insurance showing proof of required coverage and Additional <br /> Insured Endorsement; <br /> (b) AML Contractor Ownership and Control Form, if required; <br /> (c) Three-Way Agreement for Reclamation Activities, if required; <br /> (d) Completed W-9 form, (for contractors new to the State); <br /> (e) Completed EFT Form- required as of 4/15/2018. <br /> Please consult Articles 2-7 of the General Conditions. <br /> If the above documents are not submitted properly by the date specified in the award letter, the <br /> Division of Reclamation, Mining and Safety may elect to award the project to the next lowest <br /> bidder. <br /> INSURANCE <br /> A. The Contractor shall obtain and maintain, at all times during the term of this contract and <br /> require Subcontractors to include insurance in the kinds and amounts specified below. All <br /> insurance policies required herein shall be issued by insurance companies as approved by the <br /> State. <br /> 1. Workers' Compensation Insurance as required by state statute, and Employer's <br /> Liability Insurance covering all of Contractor's or Subcontractor's employees acting <br /> within the course and scope of their employment. <br /> 2. Commercial General Liability Insurance written on ISO occurrence form CG 00 01 <br /> 10/93 or equivalent, covering premises operations, fire damage, independent <br /> contractors, products and completed operations, blanket contractual liability, <br /> personal injury, and advertising liability with minimum limits as follows: <br /> Page 3 of 9 <br />
