F. ACCIDENT REPORTING: <br /> - All accidents and injuries must be immediately reported. <br /> - The contractor shall investigate all such occurrences and develop an action plan <br /> to prevent their recurrence. <br /> - All investigations shall be documented. <br /> G. FIRST AID: <br /> - First aid kits meeting MSHA (CFR 75.1713-7) requirements shall be maintained in <br /> a sanitary condition and placed at all work sites. <br /> H. COMMUNICATION: <br /> - All work areas shall have a method to communicate with personnel at all times. <br /> - Personnel will never be allowed to work alone without a secure means of <br /> communication and periodic checks by another party. <br /> I. EQUIPMENT OPERATION: <br /> - Seatbelts shall be worn at all times. <br /> - Equipment always has the right of way. <br /> - Safe parking procedures (chock blocks, etc.) shall be used at all times. <br /> - Be aware of all overhead physical and electrical hazards. <br /> - All lights, horns, brakes, wipers, back-up alarms and other safety equipment <br /> shall be fully operational at all times. <br /> - Each piece of equipment shall have at least one portable fire extinguisher. <br /> - All mobile equipment, with the exception of pick-up trucks with unrestricted <br /> rearview, will be equipped with back-up alarms. <br /> - Mobile equipment shall be equipped with roll-over- protection (ROPS) and/or <br /> falling object protection structures (FOPS) as required by MSHA regulations. <br /> - All machinery shall be secured to ensure zero mechanical potential prior to <br /> work being performed on the machinery. <br /> - Equipment shall be maintained in safe condition. <br /> - Inspect hydraulic and pneumatic lines and couplings daily. <br /> J. ELECTRICAL SAFETY: <br /> - Electrical lock and tag procedures are to be used at all times. <br /> - Be aware of all overhead electrical hazards. <br /> K. SAFE WORK IN ELEVATED AREAS: <br /> - Fall protection shall be a full body harness and shock- absorbing lanyard. <br /> - Wire rope lanyards are required for welding operations. <br /> - All personnel are to be tied off if working where a fall hazard exists. <br />
