SAFETY, HEALTH, & ENVIRONMENTAL ACTION PLAN RESPONSE ITEMS <br /> HANNA BOND FORFEITURE RECLAMATION PROJECT <br /> The following is a summary list of items that must be addressed in writing by the <br /> CONTRACTOR. Each employee is to read and sign the plan as part of the project <br /> orientation prior to starting work on the project. <br /> A. SITE ACCESS <br /> No person may enter the job site without understanding the hazards at <br /> the site. Hazards on this job include, but are not limited to, highwalls, X <br /> unstable banks and standing water. <br /> B. SUBSTANCE ABUSE <br /> Alcohol and drugs are not allowed on the job site. X <br /> C. PROACTIVE SAFETY <br /> The CONTRACTOR shall be the safety representative responsible for X <br /> safety and health at the work site. <br /> Daily safety meetings and safety inspections shall be conducted and X <br /> documented <br /> D. GENERAL SAFETY STANDARDS <br /> Hard hats and reflective vests are required to be worn at all times unless X <br /> inside equipment. This equipment must meet ANSI standards. <br /> Trenches and other excavations more than three feet deep must be <br /> properly shored or laid back to a 1 :1 slope before personnel are X <br /> allowed to enter. Care must be taken when working around highwalls <br /> and standing water. Banks may be unstable and subject to collapse. <br /> E. ACCIDENT REPORTING <br /> All accidents and injuries must be immediately reported to the <br /> Colorado Division of Reclamation, Mining and Safety project manager X <br /> and documented. <br /> F. FIRST AID <br /> First aid kits meeting MSHA (CFR 75.1713-7 or 77.1707) requirements shall X <br /> be maintained in a sanitary condition and placed at all work sites. <br /> G. COMMUNICATIONS <br /> All work areas shall have a method to communicate with other X <br /> personnel at all time.
