Accident investigation reports will be filed <br /> under the following instances: <br /> • Job-related injuries and illnesses. <br /> • Accidents resulting in significant property damage. <br /> • Accidents in which there may have been no reportable injury or property damage, but <br /> which have a high probability of recurring with at least a moderate risk to personnel or <br /> property. <br /> 9.0 STANDARD SAFE WORK PRACTICES <br /> Stop Work Authority <br /> Each individual working on the project is responsible for adherence to the safety <br /> requirements of this document and every worker has the authority and the responsibility <br /> to enforce its provisions. This authority includes calling a "stop work" for imminent <br /> danger and Stop Work for poor performance. Training in "Stop Work" authority and <br /> responsibilities will be provided to all project personnel as part of Contractor/Vendor <br /> Orientation. Work will be resumed upon approval in accordance with the procedure. <br /> The following general safe work practices apply: <br /> • Eating and drinking are prohibited in contaminated or potentially contaminated areas, <br /> or where there is a possibility for the transfer of contamination. <br /> • Contact with potentially contaminated substances should be avoided.
