STORMWATER MANAGEMENT PLAN MORRISON QUARRY <br /> REVISION NO.8 1 August 2022 CERTIFICATION NO.COG500320 <br /> 8.2.2 ANNUAL REPORT <br /> An annual report must be submitted to CDPHE WQCD for the reporting period January <br /> 1st though December 31st. A copy of the annual report form is included in Appendix <br /> C. The report shall include, but not limited to, the following: <br /> • Name of permittee, address and phone number <br /> • Contact person name, title and phone number <br /> • Facility name and physical address <br /> • Permit certification number <br /> • Summary of inspection dates <br /> • Corrective action documentation <br /> • Status of any outstanding corrective actions <br /> A signed copy of the annual report must be received by WQCD by February 28t" of <br /> the following year. Annual reports shall be submitted to the following address: <br /> Colorado Department of Public Health and Environment <br /> Water Quality Control Division, WQCD-P-B2 <br /> 4300 Cherry Creek Drive South <br /> Denver, CO 80246-1530 <br /> 8.2.3 OTHER NOTIFICATIONS <br /> Notifications will also be provided to WQCD if any of the following occur: <br /> • CHANGE IN DISCHARGE - The permittee shall give advanced notice to the <br /> Division, in writing, of any planned physical alterations oradditions to the permitted <br /> facility. <br /> • TRANSFER OF OWNERSHIP-The permittee shall give notice, in writing, 30 calendar <br /> days in advance of the proposed transfer date. <br /> • NON-COMPLIANCE - Please refer to Part II.A.3 of the permit for details on non- <br /> compliance notification. <br /> • SPILLS-The WQCD will be notified of anyspill ordischarge of oil or other substances <br /> that may cause pollution of the waters of the state. The WQCD will be notified <br /> verbally within 24 hours of the time that facility management becomes aware of <br /> 29 <br />
