Chain of custody form(s) will be <br /> completed and sent with the samples for each shipment. If multiple coolers are sent to a single laboratory on <br /> a single day,form(s) will be completed and sent with the samples for each cooler. <br /> The chain-of-custody form will identify the contents of each shipment and maintain the custodial integrity of <br /> the samples. Generally, a sample is considered to be in someone's custody if it is either in someone's physical <br /> possession, in someone's view, locked up, or kept in a secured area that is restricted to authorized personnel. <br /> Until the samples are shipped, the custody of the samples will be the responsibility of Pinyon. The sampling <br /> team leader or designee will sign the chain-of-custody form in the "relinquished by" box and note date, and <br /> time. <br /> Sample Packaging and Shipping <br /> The following outlines the packaging procedures that will be followed.As a first choice, Pinyon will hand-deliver <br /> the samples to Origins in Denver, Colorado. In the event samples cannot be submitted to Origins within three <br /> days of sample collection,the samples will be shipped,via FedEx,from Salida, Colorado directly to the Origins <br /> facility.
