• Sample preservation (if used) <br /> • Lot numbers of the sample containers,sample identification numbers and any explanatory codes,and chain- <br /> of-custody form numbers <br /> • Name of recipient laboratory <br /> In addition to the sampling information, the following specific information will also be recorded in the field <br /> logbook for each day of sampling: <br /> • Team members and their responsibilities <br /> • Time of arrival/entry on site and time of site departure <br /> • Other personnel on site <br /> • Deviations from sampling plans, site safety plans, and QAPP procedures <br /> • Changes in personnel and responsibilities with reasons for the changes <br /> • Levels of personal protective safety gear worn and required <br /> Photographs <br /> Photographs of sampling locations and other areas of interest will be taken during field activities. This includes <br /> stockpiles that either cannot be sampled due to particle sizes not fitting into the sample jar, or stockpiles that <br /> contain apparent visible creosote staining. The photographs will serve to verify information entered in the field <br /> logbook. For each photograph taken,the following information will be written in the logbook or recorded in a <br /> separate field photography log: <br /> • Time, date, location, and weather conditions <br /> • Description of the subject photographed <br /> • Name of person taking the photograph <br /> Reporting <br /> Pinyon will prepare and submit a summary report following receipt of laboratory reports and an opportunity <br /> to validate the data. The report will include: <br /> • Sampling dates, methods, and general site observations (e.g., weather, overall site conditions and activity, <br /> site personnel) <br /> • Sample locations (graphical and narrative) <br /> • Field observation data <br /> • Laboratory analytical data (tabular and narrative) <br /> • Data comparison to screening/regulatory values <br /> Mr. Lucas West Page I I <br /> Environmental Protection Specialist <br />
