Compliance With Mine Plan R - Support Facilities Not On-site <br /> N -Other N - Special Categories Of Mining <br /> N -Processing Waste R -Topsoil <br /> COMMENTS <br /> This report documents the Division's observations taken during a complete inspection of the <br /> New Horizon North Mine on March 14, 2023. The inspection was conducted by Clayton Wein, <br /> Lucas West and Todd Jesse of the Division. Tony Tennyson represented the operator, Elk Ridge <br /> Mining and Reclamation during the inspection. The weather was clear with a temperature of 52 <br /> degrees F. The ground conditions were partly dry and muddy. <br /> AVAILABILITY OF RECORDS—Rule 5.02.4(1): <br /> The records for the New Horizon North Mine are located at the mine office of the New Horizon Mine in <br /> Nucla, CO. The records were well kept and in good condition. Please see the Availability of Records Form <br /> attached to the end of this report for more details. <br /> HYDROLOGIC BALANCE -Rule 4.05 <br /> Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br /> Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br /> Drainage—Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br /> Two ponds are located within the permit area, Pond 001 and Pond 002. Pond 001 is located in the <br /> southwest portion of the permit area. Both cells of the pond were dry at the time of the inspection(Photo 1 and 2). <br /> The embankments were stable with vegetative cover. The outlet was clear of debris. Pond 002 was holding a <br /> puddle of water in it. There was no discharge occurring from the outlet(Photo 3). The outlet was unobstructed. <br /> Both the east and south inlet to the pond were stable. Holes in the south inlet lining material have been created by <br /> deer hooves. The holes have not enlarged and no undercutting of the liner has occurred. The embankments for the <br /> pond were vegetated and stable. No erosional features were identified. <br /> Ditches on along the southern permit boundary, eastern permit boundary and the southern half of the western <br /> permit boundary convey runoff to Pond 001. The ditch was damp during the inspection. No runoff was in the <br /> ditch. The ditch was clear of debris and there were no erosional features identified. The culvert underneath the <br /> entrances to the permit area were unobstructed and had little to no sediment filled in the pipe. The ditch along the <br /> northern half of the western permit boundary conveys runoff to Pond 002.
