e. Certificates of insurance must be provided to the State within 10 working days <br /> after receipt of Award; and, <br /> f. The contractor must carry a minimum of$600,000 <br /> combined single limit. <br /> VENDOR OFFSET <br /> Pursuant to C.R.S. 24-30-202.4 (as amended), the State Controller may withhold debts owed to <br /> state agencies under the vendor offset intercept system for: (a) unpaid child support debt or <br /> child support arrearages; (b) unpaid balance of tax, accrued interest, or other charges specified <br /> in Article 22, Title 39, C.R.S.; (c) unpaid loans due to the student loan division of the <br /> Department of Higher Education; (d) owed amounts required to be paid to the unemployment <br /> compensation fund; and (e) other unpaid debts owing to the State or any agency thereof, the <br /> amount of which is found to be owing as a result of final agency determination or reduced to <br /> judgment as certified by the controller. <br /> Work Description <br /> The attached General Conditions for Purchase Orders and Purchase Order Terms and <br /> Conditions apply.to all work. <br /> The work involves constructing two grated adit closures at the London Mine. One mine opening <br /> is located at the Water Tunnel Level, north of South Mosquito Creek and the second mine <br /> opening is located south of South Mosquito Creek at the Butte Tunnel. The steel grates must be <br /> constructed in accordance with the Grated Adit Closure specifications in the General Bid <br /> Specifications, Colorado Inactive Mine Reclamation Program, 1996. The steel grating must be <br /> attached to the existing timber sets in the adits using 4-inch long, 3/8-inch diameter lag bolts on <br /> one foot centers. The bolts must be welded to the steel grating following embedment into the <br /> timber sets. <br />
