Colorado Inactive Mine Reclamation Program <br /> CLOSEOUT REPORT <br /> Diamond Mine Bond Forfeiture Project <br /> PKA-12-1209 <br /> Permit Information <br /> Permit Number: Operator Name: Mine Name: <br /> M-1986-141 Leadville Corporation—James Tiffany Diamond Mine <br /> Type of Mine : 110(2)- UG Commodity Mined: Au, Ag <br /> PRE-RECLAMATION SITE CONDITIONS: <br /> The Diamond Mine Bond Forfeiture Project is located in Lake County, Colorado, approximately four(4) miles east of <br /> Leadville. The Diamond Mine, located at 11,300' in elevation, was an underground gold and silver mining operation that <br /> was permitted in 1986. The site consists of a shaft house, dry, and shop all consolidated in one building and an adjacent <br /> hoist house. The permit was revoked and the financial warranty forfeited in February 2005. <br /> The purpose of this project was to reclaim the abandoned mine site in accordance with the reclamation plan required by the <br /> permit. <br /> The project work consisted of safeguarding several openings in the shaft house with steel grating to prohibit entry into the <br /> structure and to prevent accidental falls, the removal of all debris from the site, and Grading the waste rock pile to a 2:1 <br /> slope, revegetation with native seed, and the application of fertilizer and wood mulch. Analytical results confirmed that the <br /> material contained asbestos, but in quantities less than 1%. Meaning the vermiculite was not an asbestos containing <br /> material (ACM), defined as having asbestos concentrations greater than 1%, and was not located within a confined spaced <br /> and therefore could be mitigated with less regulation. <br /> The winning contractor, RMC Consultants, was capable and willing to mitigate the vermiculite containing asbestos and, at <br /> the request of the Project Manager, submitted a proposal in the amount of$2,175.00 to complete the abatement. This <br /> initiated the first and only Change Order for the Diamond Mine Bond Forfeiture Project. <br /> The contractor mobilized to the project site on August 16, 2011 and immediately began preparations to safeguard the <br /> openings in the shaft house with steel grating. By the end of the first week the shaft house was secured, debris was loaded <br /> into trailers and ready for disposal and the silt fence was installed along the bottom edge of the waste rock pile in <br /> preparation for re-grading. <br /> On August 22, heavy equipment arrived onsite to begin reducing the slopes from the existing 1:1 to the desired 2.5:1 slope. <br />
