Colorado Inactive Mine Reclamation Program <br /> CLOSEOUT REPORT <br /> LEVICY BOND FORFEITURE PROJECT <br /> Introduction: The Levicy project was developed to complete the reclamation requirements that were not completed by the <br /> operator under their reclamation permit. The project was completed by closing the main adit, an open shaft, a collapsing <br /> shaft, and an exploration pit. Additional reclamation activities included grading the disturbed areas, road grading to <br /> oromote non-erosive drainage across the road, and removing trash and debris from the site. <br /> Permit Information <br /> Permit Number Operator Name Mine Name <br /> M-1978-79 Roy F. Carpenter Levicy Mine <br /> Type of Mine-Hardrock Commodity Mined: Ag and Pb <br /> PRE-RECLAMATION SITE CONDITIONS <br /> The Levicy Mine site is located 5.5 miles north of Forest Road 742, on Forest Road 584 (Tellurium Creek). An <br /> application for Mining and Reclamation Activities, Limited Impact Operations permit was issued to Roy F. <br /> Carpenter in 1978. The mining permit was revoked and the financial warranty forfeited by the State of Colorado <br /> in 2004. The site has been obtained by the U.S. Forest Service, and is currently managed by the Gunnison <br /> Ranger District. The entrance to the Levicy site was unprotected from ATV users and backcountry users, <br /> presenting a safety hazard. <br /> BIDDING/CONSTRUCTION INFORMATION <br /> 'DRMS received six bids for this project. The <br /> main adit was backfilled, and the area in front of the adit was graded. The open shaft was closed using Polyurethane Foam <br /> and rock for backfill. The collapsing shaft and the exploration pit were backfilled with on-site waste materials and graded to <br /> drain. The access road was graded and drainages were constructed to promote non-erosive drainage from the road. <br /> Additionally all remnant structures, trash, and debris were removed from the site.
