The contractor must carry a minimum of $600,000 <br /> combined single limit. <br /> GENERAL BID SPECIFICATIONS <br /> The Colorado Inactive Mine Reclamation Program General Bid Specifications 1996, which <br /> include General Conditions, Standard Work Specifications, and Standard Drawings and Figures <br /> are intended to complement these Special Conditions. The Special Conditions combined with <br /> the General Bid Specifications 1996 form the complete Invitation and Bid document. Please <br /> refer to the Standard Work Specifications for all applicable types of work required. Copies of <br /> the General Bid Specifications are available from the Division of Minerals and Geology (DMG), <br /> 1313 Sherman Street, Room 215, Denver, Colorado 80203 (303) 866-3567. <br /> Work Description <br /> The Clay County shaft is located on the north side of Lake Gulch immediately south of the <br /> entrance to the Miners Mesa parking lot. Bids are requested for mobilization/demobilization, <br /> excavating the precast panel foundation, placement of two D-1 or E-1 panels, placement of <br /> one extra D-1 or E-1 panel if needed, and placement of two five foot diameter concrete pipe <br /> sections. <br /> The Project is scheduled for construction beginning October 13, 1997, and ending by October <br /> 17, 1997. <br /> The attached Purchase Order Terms and Conditions apply to all work. <br /> VENDOR OFFSET INTERCEPT SYSTEM <br /> Pursuant to C.R.S. 24-30-202.4 (as amended), the State Controller may withhold debts owed <br /> to state agencies under the vendor offset intercept system for: (a) unpaid child support debt or <br /> child support arrearages; (b) unpaid balance of tax, accrued interest, or other charges specified <br /> in Article 22, Title 39, C.R.S.; © unpaid loans due to the student loan division of the <br /> Department of Higher Education; (d) owed amounts required to be paid to the unemployment <br /> compensation fund; and (e) other unpaid debts owing to the State or any agency thereof, the <br /> amount of which is found to be owing as a result of final agency determination or reduced to <br /> judgement as certified by the controller. <br /> TASK 1: MOBILIZATION/DEMOBILIZATION <br /> This Task includes those measures necessary to move equipment and supplies onto the <br /> project location. This item also includes all labor and equipment necessary to demobilize from <br /> the site, including clean up of the site to its original or better condition.
