(b) Fully-executed Performance and Payment Bonds, accompanied by <br /> appropriate Power of Attorney, if required; <br /> (c) Certificates of Insurance showing proof of required coverage and Additional <br /> Insured Endorsement; <br /> (d) Project Schedule; <br /> (e) AML Contractor Ownership and Control Form; <br /> (f) Minority(MBE)/women (WBE) Business Participation form; <br /> (g) Three-Way Agreement for Reclamation Activities, if required by the Special <br /> Conditions; <br /> (h) A list of all subcontractors to be used on the project, the work tasks and dollar <br /> value to be subcontracted to each, and <br /> (i) Completed W-9 forms, if required (for contractors new to the State). <br /> 0) Completed EFT Form <br /> Please consult Articles 2-7 of the General Conditions. <br /> If the above documents are not submitted properly by the contract signing date, the Division of <br /> Reclamation, Mining and Safety may elect to award the contract to the next lowest bidder. Bidders <br /> are advised to keep close contact with CDRMS during this period. <br /> THREE-WAY AGREEMENT <br /> A three way "Agreement for Reclamation Activities" is not required for this job. Please see Article <br /> 4 of the General Conditions for the applicable insurance requirements. <br /> INSURANCE <br /> A. The contractor shall obtain at his own expense, and maintain at all times during the term of <br /> this contract, insurance listed below. Proof of insurance must be submitted on certificates <br /> showing the following minimum coverage: <br /> 1) Worker's Compensation Insurance as required by state statute, and Employer's <br /> Liability Insurance covering all of contractor's employees acting within the course and <br /> scope of their employment. <br /> 2) Commercial General Liability Insurance written on ISO occurrence form CG 00 01 <br /> 10/93 or equivalent, covering premises operations, fire damage, independent <br /> contractors, products and completed operations, blanket contractual liability, personal <br /> injury, and advertising liability with minimum limits as follows: <br /> a. $1,000,000 each occurrence; <br /> b. $1,000,000 general aggregate; <br /> C. $1,000.00 products and completed operations aggregate; and <br /> d. $50,000.00 any one fire. <br /> If any aggregate limit is reduced below $1,000,000 because of claims made or paid, <br /> the contractor shall immediately obtain additional insurance to restore the full <br /> aggregate limit and furnish to the State a certificate or other document satisfactory to <br /> the State showing compliance with this provision. <br /> 3) Automobile Liability Insurance covering any auto (including owned, hire and non- <br /> Page 5 of 16 <br />
