The site is in final reclamation,and not active
the day of the inspection. The weather was cloudy and muddy. Jason Carver was contacted,however he did not
accompany the inspection. Six Photos accompany this report to illustrate the observed site conditions.
BACKFILL and GRADING—Rule 4.14
Contemporaneous Reclamation 4.14.1;Approximate Original Contour 4.14.2;High wall Elimination 4.14.1(2)(f);
Steep Slopes 4.14.2,4.27;Handling of Acid and Toxic Materials 4.14.3; Stabilization of Rills and Gullies 4.14.6:
The area located near the Johnson Property stockpile area known as the backfill area was observed. The
area appeared stable at the time of the inspection and no evidence of settling, slumping or significant erosion was
noted. The backfill area can be seen in Photo One. In response to recent precipitation events,small areas of
ponded water were observed.
HYDROLOGIC BALANCE -Rule 4.05
Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7,4.05.10;
Diversions 4.05.4;Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13;
Drainage —Acid and Toxic Materials 4.05.8;Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18:
Due to the muddy ground conditions not all water impoundments and ditches were observed. The
inspection of these items was limited to the accessible areas.
Around the Facilities area all ditches and water conveyances were observed to be in good working order and free
from debris. Pond 018 located towards the South end of the facilities area contained standing water in response to
recent precipitation events. The water level, seen in Photo Two,was low and no evidence of discharge or
migration was noted. The banks of the pond are well vegetated and appeared stable at the time of the inspection.
Additionally,at the facilities area all areas that previously stored hazardous materials that would require secondary
containment were verified to contain no hazardous materials. The only area that contains very small quantities of
oils and fluids is located in the pole barn just South of the office building. This area can be seen in Photo Three,
all items are stored on a spill pallet with adequate containment.
Number of Partial Inspection this Fiscal Year: 3
Number of Complete Inspections this Fiscal Year: 1
Page 2 of 6
