The site is in final reclamation,and not active
the day of the inspection. The weather was clear and dry.Mindy Sickels of Elk Ridge Mining and Reclamation
Accompanied the inspection and represented the Operator. Six Photos accompany this report to illustrate the
observed site conditions.
HYDROLOGIC BALANCE -Rule 4.05
Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7,4.05.10;
Diversions 4.05.4;Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13;
Drainage —Acid and Toxic Materials 4.05.8;Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18:
All fuel and hazardous materials storage areas were observed as part of this inspection. The main above
ground fuel tank that has been on site has been removed,including the pump and all delivery lines. The secondary
containment structure is still present,however will be removed in the near future. The footprint of the tank area
including the pump station showed no signs of leakage or spillage during the removal process. The area where the
tank used to be can be seen in Photo One. The used oil tanks located adjacent to the shop building were observed
and verified to be empty. These tanks have been decommissioned and not used in some time,however still
present on site. With the decommissioning of the used oil tanks and the removal of the bulk fuel storage tank,the
only remaining area that hazardous materials are stored is the pole barn to the south of the office. This small area
contains spill pallets where small quantities of hydraulic fluid and various pieces of equipment containing oils are
stored. All items stored on the pallets are in good condition and can be seen in Photo Two. No evidence of
significant spillage or loss of containment was noted.
TOPSOIL—Rule 4.06
Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3; Redistribution 4.06.4:
All topsoil stockpiles were observed during this inspection. The Johnson Property storage area piles,
including the recently relocated piles have established vegetative cover and appeared to be stable. All piles were
well marked, and the ring berms around the perimeter of the piles are in good condition. No evidence of erosion
or transport of topsoil was noted. Examples of the piles can be seen in Photos Three and Four.
Number of Partial Inspection this Fiscal Year: 1
Number of Complete Inspections this Fiscal Year: 0
Page 2 of 6
