Compliance With Mine Plan N - Support Facilities Not On-site <br /> N -Other N - Special Categories Of Mining <br /> N -Processing Waste R -Topsoil <br /> COMMENTS <br /> This was a partial inspection conducted by Lucas West of the Colorado Division of Reclamation,Mining and <br /> Safety as part of the normal monitoring program for permitted sites. Jordan McCourt accompanied the inspection <br /> and represented the Operator. Normal activies were takaing place at the mine during the inspection. at the King II <br /> area,the stackers were active and trucks were bineg loaded at the time of the inspection. No activities were being <br /> conducted at the King I area. The weather was clear and dry,Nine Photos accompany this report to illustrate the <br /> current site conditions. <br /> HYDROLOGIC BALANCE -Rule 4.05 <br /> Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7,4.05.10; <br /> Diversions 4.05.4;Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br /> Drainage —Acid and Toxic Materials 4.05.8;Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18: <br /> All bulk storage of fuels,oils and other hazardous materials is located in a pole barn near the parking lot at <br /> the King II facility. The hazardous materials storage area is well kept,neat and orderly. All items and tanks <br /> stored in this area are within proper secondary containment structures and no evidence of spills or leaks were <br /> noted. The facility and its contents can be seen in Photos One and Two. <br /> SUPPORT FACILITIES -Rule 4.04: <br /> At the King I facility,two separate Refuse Piles are located. The condition of the Upper Refuse Pile is <br /> largely unchanged as the Lower Refuse Pile has been primarily used recently. The Upper Pile is sloped to <br /> promote positive drainage with erosion control berms installed to prevent erosion. The pile is well compacted and <br /> appears to be within the design specifications. No standing water or evidence of impounded storm water was <br /> noted. The Upper Pile can be seen in Photo Three. <br /> Photos Four and Five show the Lower Refuse Pile. At the time of the inspection,the pile was compacted and has <br /> been re-sloped to a gentler grade to prevent erosion and sedimentation. The lower half of the pile as well as the <br /> interface between the pile and the access road was recently re-worked and is in excellent condition. The Operator <br /> has done an exemplary job of reworking the pile and repairing the small erosional features noted in previous <br /> inspection. <br /> Number of Partial Inspection this Fiscal Year: 8 <br /> Number of Complete Inspections this Fiscal Year: 3 <br /> Page 2 of 8 <br />
