Mr.Bill Tezak
Apri18,2022
Page 2 of 11
parcels shown by this map should be labeled/numbered to correlate with ownership information provided in the
Table described below. (A similar numbered map was provided during adequacy,but may need to be updated if
permit boundary is updated.)
The provided parcel/claim map should correlate to a table containing the following column headings(at a
minimum): Parcel/Claim ID; patented/unpatented claim or privately owned; Surface owner;Mineral Rights
owner; access provided(for property within the permit)as required by Exhibit N; and clear identification of the
document provided in the application which provides access to that property. The approved BLM document will
serve as access to areas where BLM holds surface and mineral access.
APPLICATION FORM:
The application forms will need to be modified to accurately reflect the final proposed acreages for this
conversion.
The most recent application form submittal(with reduced acreage totals) again left the pre-andpost-mining
land use boxes unchecked—please review all submittals for completeness and internal consistency. DRMS
notes thatpreviously submitted text stated thatpre-and post-mining land use should be identified as
"rangeland':
6.2 GENERAL REQUIREMENTS OF EXHIBITS
6.2.1 General Requirements
(1)This Rule provides for the guidelines for, and information requirements of, each Exhibit required to be
submitted with the permit application,as specified according to Section 6.1.
(2)Maps and Exhibits Maps, except the index map,must conform to the following criteria:
(a) show name of Applicant;
(b)must be prepared and signed by a registered land surveyor,professional engineer, or other qualified
person;
(c)give date prepared;
(d)identify and outline the area which corresponds with the application; The full extent of the proposed
permit must be shown,and any distinction between permit boundary and affected area (if there is to be
one)must be made clear on all applicable figures. The most recent map submittals contained
inconsistent permit boundary depictions, and no clear depiction of affected area as discussed.
(e)with the exception of the map of the affected lands required in Section 34-32.5-112(2)(d),C.R.S.
1984,as amended, shall be prepared at a scale that is appropriate to clearly show all elements that are
required to be delineated by the Act and these Rules. The acceptable range of map scales shall not be
larger than 1 inch=50 feet nor smaller than 1 inch=660 feet.Also,that a map scale,appropriate legend,
map title,date and a north arrow shall be included.
Please make sure all maps/figures submitted comply fully with the requirements of 6.2.1..
