The site is in final reclamation,and not active
the day of the inspection. The weather was clear and dry.Mindy Sickels of Elk Ridge Mining and Reclamation
Accompanied the inspection and represented the Operator. Eight Photos accompany this report to illustrate the
observed site conditions.
HYDROLOGIC BALANCE -Rule 4.05
Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7,4.05.10;
Diversions 4.05.4;Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13;
Drainage —Acid and Toxic Materials 4.05.8;Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18:
All fuel and hazardous materials that are stored at various locations within the facilities area were
observed during the inspection. The site contains one bulk fuel storage tank,two decommissioned used oil storage
containers and a miscellaneous storage area. The bulk fuel storage tank is located near the shop facility,used for
vehicle and equipment fueling. The tanks secondary containment device is in excellent condition and no evidence
of spills or leakage was observed. The transfer lines and filling station also show no signs of leaks or spills. The
tank and filling station can be seen in Photo One. Located adjacent to the shop,on the south side,are two
decommissioned used oil containers that were in use while the mine was active. Though they are decommissioned
and no longer in use,the tanks are in excellent condition and can be seen in Photo Two. Near the office building
is the small pole barn that houses the miscellaneous storage area. Typically various oils,hydraulic fluids and other
hazardous materials are stored on spill pallets. Recently,almost all of those items have been removed from the
site. At the time of the inspection only a few small tanks and small pieces of equipment are being stored on the
spill pallets. The secondary containment of the spill pallets is in good condition and far exceeds the volume of
materials stored. Photo Three shows the spill pallet with materials.
TOPSOIL—Rule 4.06
Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3;Redistribution 4.06.4:
The topsoil storage area adjacent to the shop facility and its various piles were observed. All topsoil
stockpiles in this area were stable at the time of the inspection and no evidence of erosion or sedimentation was
Number of Partial Inspection this Fiscal Year: 6
Number of Complete Inspections this Fiscal Year: 3
Page 2 of 7
