PERMIT#:M-1979-089 <br /> INSPECTOR'S INITIALS:ACY <br /> INSPECTION DATE:February 22,2022 <br /> The following inspection topic(s)were identified as having a Problem (PB), which includes correction actions <br /> and a deadline whereby the Operator must demonstrate compliance with the conditions of the Permit and the <br /> requirements of the Act and Rules. Failure to address the corrective actions by the deadline may cause the <br /> Division to escalate the Problem to a Possible Violation (PV) and schedule the issue for formal hearing before <br /> the Mined Land Reclamation Board (Board). <br /> INSPECTION TOPIC: Gen. Compliance With Mine Plan <br /> PROBLEM: The current mine and reclamation plans needs to be updated and clarified pursuant to C.R.S. 34- <br /> 32.5-112 (1)(c)(VI) and 34-32.5-116 (1) . The operator must provide sufficient information to describe or <br /> identify how the operator intends to conduct the operation. And the operator must follow approved reclamation <br /> plan or provide sufficient information to describe or identify how the operator intends to conduct reclamation. <br /> CORRECTIVE ACTIONS: The operator shall submit a Technical Revision, with the required$216 revision <br /> fee,to update and clarify the current mine and reclamation plans and maps to reflect existing and proposed <br /> activities by the corrective action date. <br /> CORRECTIVE ACTION DUE DATE: 6/01/22 <br /> INSPECTION TOPIC: Signs &Markers <br /> PROBLEM: The affected area boundary markers are missing or incorrectly placed. This is a problem for failure <br /> to maintain boundary markers around the affected area as required by Section 3.1.12(2) of the rule. <br /> CORRECTIVE ACTIONS: The operator shall conduct a survey and replace the boundary markers in the <br /> correct location(s). The operator shall provide proof to the Division that this has been done by the corrective <br /> action date. <br /> CORRECTIVE ACTION DUE DATE: 5/09/22 <br /> OBSERVATIONS <br /> This inspection was conducted as part of the Colorado Division of Reclamation, Mining and Safety's normal <br /> monitoring program. The 23 Road Pit is a 112 permitted site located off River Road in Grand Junction <br /> Colorado, located within Mesa County. Jim Doody represented the Operator and accompanied Amy Yeldell of <br /> the Division on the inspection. <br /> The permitted area is 82 acres, of which nearly all has been disturbed. The post-mining land use of the site is a <br /> combination of commercial industrial for the backfilled areas and recreation/wildlife habitat surrounding the <br /> ponds. <br /> The pit was inactive at the time of the inspection. According to Mr. Doody the pit is nearly mined out and was <br /> last active in the fall (2021). There is a scale and scale house on site however no heavy equipment was present. <br /> A few small stockpiles remain throughout the site. A fuel tank was located on site within the laydown yard <br /> area. <br /> Page 2 of 8 <br />
