specifically state the actions to be taken by CONTRACTOR, including crew and <br /> subcontractor(s) order to address the safety concern. <br /> Measurement and Payment <br /> There will be no measurement for payment. Payment for this Task will be reflected in a lump <br /> sum quote under Task 2.0 on the Bid Schedule. This lump sum quote will cover all costs for <br /> developing and implementing the Project Safety Plan or SHEAP, including writing and revising <br /> the plan as required, provision and maintenance of safety equipment, conduct of daily safety <br /> meetings and all other items necessary and incidental to meeting the specifications of this task <br /> and of the Project Safety Plan or SHEAP. Payment shall be made as this task is completed and <br /> approved by the PROJECT MANAGER. Payment for this task will be made only on the final <br /> (before retainage) invoice. <br /> TASK 3.0 BERM CONSTRUCTION <br /> The CONTRACTOR shall construct an earthen berm along a portion of the eastern boundary of <br /> the old permit area as directed by the PROJECT MANAGER to prevent storm water from <br /> crossing the county road (see Figure 2). <br /> The berm will range between zero feet to three feet(0 ft-3.0 ft)dem(depending on topography <br /> between two hills) so that the top of the berm will be a minimum height of two and one-half feet <br /> (2.5 feet) above the bottom elevation of the central culvert under the county road north of the <br /> berm location. Approximate dimensions of the berm will be one hundred and ninety feet(190 <br /> feet) long by twenty feet(20 feet)wide at the top, with no slopes greater than 3(h):I(v) (see <br /> Figure 3). It is estimated that three hundred and seventy cubic yards (370 cubic yards)of <br /> material will be needed. <br /> Placed material shall be compacted with mechanical compaction techniques approved by the <br /> PROJECT MANAGER. Thorough compaction shall consist of a minimum of two (2)passes <br /> over all areas so that the marks overlap one another. Equipment shall be tracked forward in a <br /> single pass and return along the same path. <br /> A stockpile of overburden material is located on-site approximately four hundred feet(400 <br /> feet)from the berm location that can be used construction material. If available, larger sized <br /> material will be placed along the western side of the berm to armor against erosion. <br /> Measurement and Payment <br /> No measurement for payment shall be made for berm construction. Payment will be lump sum <br /> for Task 3.0 on the Bid Schedule. The price bid for berm construction will cover all costs for <br /> completing the activities required under this Task. Payment shall be as this task is completed and <br /> approved by the PROJECT MANAGER. Such payment shall be considered full compensation <br /> for all labor,materials, equipment,transportation and all other items and materials necessary and <br /> incidental to the completion of Task 3.0. <br /> Page 8 of 14 <br />
