c) name(s), address(es),telephone number(s) and email address(es) of the Operator's contact <br /> person for additional information and follow-up by the Office: <br /> d) monitoring and analysis that are necessary to evaluate the situation and corrective actions, <br /> copies of all pertinent data; and <br /> e) results of the Operator's investigation to assess the conditions or circumstances that created the <br /> emergency situation, and what corrective or protective measures will be taken to prevent a <br /> similar event form occurring in the future. <br /> 8.3 Emergency Response Plan <br /> This ERP is being drafted prior to ground breaking of mine development. The mine Health and Safety <br /> Department personnel hierarchy is yet to be determined,therefore the following is an anticipated ERP <br /> and will be updated as the mine develops. The mine's Health and Safety Manager will develop an ERP <br /> that is compliant with US Mine Safety and Health Administration (MSHA). In addition, Fremont County <br /> Planning and Zoning Department require a Fire Protection Plan as a component of the Conditional Use <br /> Permit application. <br /> 8.3.2(a) Designated Personnel <br /> Mine personnel and hierarchy are speculative as of the date of this application. Anticipated personnel <br /> responsible for responding to an emergency may include the Mine/Mill Manager, Mine Foreman, Health <br /> and Safety Manager and Underground Mine Manager. The names and contact information will be <br /> provided upon determination. <br /> 8.3.2(b) Emergency Response Procedure <br /> Any emergency situation will be successfully resolved by a phased response consisting of <br /> notification, operations, and remediation.These three phases will be implemented in concert or <br /> sequentially depending upon the specific situation and available personnel. <br /> Phase 1: Notification <br /> The first person to arrive at the location of an emergency situation becomes the First <br /> Responder to the incident and assumes responsibility for the subsequent emergency <br /> response until they are relieved by an Emergency Coordinator, or voluntarily relinquish <br /> their authority to a more qualified person.The First Responder can be any DGM employee <br /> or contractor's employee (e.g. chemical delivery contractor). <br /> The primary responsibility of the First Responder in an emergency situation is the prompt <br /> notification of other site personnel.The First Responder shall immediately relay an incident <br /> evaluation to at least one other person prior to initiating the operations phase during an <br /> emergency response.The incident evaluation must include the following information, at a <br /> minimum. <br /> a) The location of the incident. <br /> b) The nature of the incident. <br /> c) The extent of injury, if applicable. <br /> d) The type of material spilled, if known and applicable. <br /> e) The physical extent of the spill area, if applicable. <br /> 82 <br />
