The Division requires the Applicant submit the application pages for a 111(1)(a)permit. <br /> <br />The Division received a $400.00 fee for an 111(1)(b) permit. As per the application requirements the fee <br />for a 111(1)(a) permit is $898.00. <br /> <br />2. The Division requires the Applicant to submit the difference between the sum submitted <br />($400.00), and the required fee. Please submit an additional $498.00 with the 111(1)(a) application. <br /> <br />6.3.3 Exhibit C Mining Plan <br />3. Please update as necessary according to permit 111(1)(a) requirements. <br />4. Specify the thickness of the deposit to be mined. <br />5. Specify the dimensions of any significant disturbances to the land surface such as pit excavations, <br />mine benches, impoundments, stockpiles, waste rock disposal areas, etc. <br />6. Please provide dimensions (length and width), for the farm road used for access and referenced <br />in section 6 of this rule in the application. <br />7. Specify how you will comply with applicable Colorado water laws and regulations governing <br />injury to existing water rights. <br />8. If refuse and acid or toxic producing materials are exposed during mining, describe how they <br />shall be handled and disposed of in a manner that will control unsightliness and protect the drainage <br />system from pollution. <br />9. Describe what measures will be taken to: <br />o minimize disturbance to the hydrologic balance, <br />o prevent off-site damage, <br />o provide for a stable configuration of the reclaimed area consistent with the proposed future land <br />use. <br />10. Specify whether the deposit/ore will be processed on-site. Processing includes crushing, <br />screening, washing, concrete or asphalt mixing, leaching or milling. If the deposit/ore will be processed, <br />then describe the nature of the process, facilities and chemicals utilized. The process area and any <br />structures must be described on Exhibit E - Map. <br />11. Identify the primary, and if applicable, secondary and incidental commodities to be <br />mined/extracted by the proposed operation. <br />12. Drainage Control: Describe the measures used to divert upland drainage away from the site both <br />during and after operation. This must include design details demonstrating the capacity of ditches and <br />impoundment structures to contain operating solutions and the volume of water generated by a one <br />hundred (100) year 24-hour rainfall event. <br /> <br />6.3.4 Exhibit D Reclamation Plan <br />13. Specify at what point in the mining plan and to what depth(s) overburden will be replaced in <br />relation to ongoing extraction. <br /> <br />Sediment control and protection of hydrologic balance were not explicitly addressed in the Mine Plan. <br />Reclamation of the topsoil pile was also not explicitly addressed. <br /> <br />14. Specify the reclamation treatment of any waste rock dumps, ditches, sediment control facilities, <br />specified in your mine plan but not previously addressed in the Reclamation Plan narrative. These features <br />must be shown on Exhibit E - Map.
